11/15/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax verbalized to QMHPA her morning was well. Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA discussed with Ms. Broadnax that it is necessary that she understand the concept of presenting a healthier outcome in situations that she doesnât have any control over. QMHPA reviewed the importance of being able to appropriately control her anger and maintaining good mood stability. QMHPA educated Ms. Broadnax on the opponents of anger and discussed various situations and things that have made her extremely upset with her sister and others that she may come in contact with. QMHPA reviewed learned techniques such as deep breathing and counting up and down when she feels as though she is going to become angry to prevent from displaying inappropriate behaviors. QMHPA implemented techniques to assist Ms. Broadnax with relieving her stress which included exercising, reading a book of her interest, breathing techniques as well as guided imagery techniques which allows her to think of positive situations. QMHPA also encouraged Ms. Broadnax to work on replacing her negative feelings and thought with those that are positive. QMHPA will continue to provide Ms. Broadnax with ways to cope in stressful situations through role-playing and allowing Ms. Broadnax to discuss her thoughts, feelings, and emotions of sadness and frustrations during counseling sessions. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax to QMHPA that utilizing techniques was hard because she tends to react before she actually takes time out to think about the situation. As mentioned in the behavior, Ms. Broadnax indicated to QMHPA, âI do not like that that girl and nothing is going to make me like herâ (situation that occurred within the home with her and another). Ms. Broadnax actively engaged in conversation in regards to stress reduction and stated that she needed to learn to utilize techniques because she tends to become stressed on a consistent basis when it comes situations that she doesnât have control over. Ms. Broadnax stated that she would learn to implement strategies so that she can relieve her feelings of frustration and anxiety in addition to continuing to adhere to her medication management to maintain her mental health stability. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
